Fix connection line source position, improve sag, add smart spawning

Fixes: - Source position now correctly uses offsetX/offsetY from where connection icon was clicked (not hardcoded to task item width) - Both established connections and dragging connection use proper offsets Improvements: - Increased sag for more natural rope-like appearance (25% of distance + 20px min) - Max sag increased to 120px for longer distances Smart spawning for new task items: - New items spawn in non-overlapping positions - Prefers placing to the right of rightmost item - Falls back to below bottom-most item - Grid search for complex layouts - 50px spacing between items
