Build a custom lead-capture form your organization can attach to one or more markets.
Before you start
The Manage CRM Settings right. Building the form doesn't require a market to exist yet — attaching it to one is a separate step (see How to Assign Custom Forms to a Market).
Steps
- Go to Settings → Client Relations Manager → Custom Forms.
- Click + Create Form.
- Fill in Form Title (required) and Form Description (optional).
- Click + Add Stack to add your first stack — a stack is a horizontal row/group of fields on the form. Give it an optional description.
- Inside a stack, click + Add Field and pick a field type from the Select Field Type dialog:

- Configure the field:
- Field Title — the label shown to whoever fills out the form.
- Field ID — a unique identifier. Must be lowercase letters/digits/hyphens/underscores only, can't start with a digit or a hyphen/underscore, and can't repeat elsewhere on the form.
- Nullable and Display on Intake toggles.
- Type-specific options — e.g. Max Characters for Text, Rows for Text Area, Options for Dropdown/Radio/List, a default value where applicable.
- Click Save Field. Repeat for as many fields and stacks as the form needs — drag a stack or a field to reorder it.
- Click Save Form (the dialog blocks saving if any field has a title, ID, or duplicate-ID error — look for a Has errors chip on the offending field).
What you'll see
The form is saved and appears in the Custom Forms list, ready to attach to one or more markets. Empty stacks (no fields) are dropped automatically when you save.
